Networked Care App for caregivers launched in Singapore


SAP SE and Sierra Solutions on Nov 28 announced the launch of eCareApp, a Networked Care application built by Sierra Solutions on SAP’s HANA Cloud Platform.

The app has been developed with caregivers in mind, providing best practices and continuity of patient care between doctor visits. The aim is to improve the overall quality of care provided to geriatric patients in Singapore, through a personalised care plan for caregivers and physicians.

eCareApp demonstrates the potential of connected care applications in providing support to caregivers and healthcare professionals.

The app’s capabilities include learning assistance around a family member's condition, sharing knowledge via in-app communication channels, shared journal logs, tracking daily progress, and more. In-app visual guides and historical records ensure such information can be easily understood and discussed with doctors, making it easier for hospitals to provide customised care plans for outgoing patients, whose families can then take over the assigned care tasks.

The launch follows a joint pilot project conducted across 10 families in the first half of 2016 in partnership with ACE Seniors, a Singapore-based social enterprise and interest group that offers a holistic approach towards successful ageing.

The pilot saw participation from local care organizations including Alzheimer’s Disease Association Singapore (ADA), Centre For Seniors (CFS), AWWA, and Metta Alumni Care (Metta).

In cases such as dementia, educating the network of caregivers on what to expect and how to respond becomes vital to good patient care. Early feedback from the pilot phase has shown that a majority of caregivers find communications between family members a significant challenge. 

Key patient data is stored and shared between caregivers and healthcare professionals to enable better clinical decisions and record vital information necessary for diagnosis.

“Our joint effort with Sierra on eCareApp has from day one had the power of software and technology at its heart – with the aim to improve the connected care experience. We’re already seeing the app’s incredible impact on the lives of elderly users and their caregivers.

“Since our initial pilot, we’ve worked tirelessly to improve and refine the solution based on hard data and insights. We believe strongly in the need for ongoing innovation, collaboration, and co-creation of new digital technologies in the healthcare space to support an ageing population in Singapore,” said SAP Asia Pacific region Healthcare director Andy David.

“We look forward to leveraging on the benefits, efficiencies, and education capabilities in the emerging connected care space that eCareApp has to offer. In partnership with Centre for Seniors, we’re committing to a large-scale implementation throughout our community groups. What we really like is that it’s designed with the caregivers in mind, allowing them to access information in real time to care for their loved ones,” said ACE Seniors executive chairman Dr. Bee Wan Tan.

In the last decade, Singapore’s resident population has grown older according to Singstat. As at end-June 2016, the proportion of residents aged 65 years and over has increased from 8.4% in 2006 to 12.4% today.

The region is experiencing growing demand for care related to conditions such as diabetes, cardiovascular disease, non-communicable diseases (NCDs), cancer, and dementia – all of which require long-term monitoring and seamless information sharing between patients, caregivers, and healthcare professionals. Providing better support to home caregivers is an important but often forgotten part of the care continuum.
